AstraZeneca is advancing its oral GLP-1 receptor agonist, elecoglipron, into a comprehensive Phase 3 development program. This move follows promising results from two Phase 2b studies that showcased significant weight reduction and effective diabetes management.

The VISTA trial, involving 310 adults classified as obese or overweight with at least one related comorbidity, revealed substantial efficacy. Participants receiving 75 mg of elecoglipron experienced an average body-weight reduction of 10.5% after 26 weeks, a stark contrast to the 0.6% observed in the placebo group. This weight loss further extended to 11.8% by week 36, compared to 0.3% for placebo. Additionally, a remarkable 88.8% of patients on elecoglipron achieved at least a 5% weight loss, meeting a co-primary endpoint. Exploratory cardiometabolic measures also improved, with reductions in blood pressure and C-reactive protein, a key indicator of systemic inflammation. Similarly, the SOLSTICE trial, which enrolled 404 adults with Type 2 diabetes, demonstrated elecoglipron's ability to lower HbA1c levels by an average of 1.9% at 26 weeks, versus 0.2% for placebo. The majority of treated patients, 90%, reached HbA1c levels below 7%, and 85% achieved levels of 6.5% or lower. Furthermore, these patients experienced an average body-weight reduction of 7.7%, compared to 1.7% in the placebo group.

Across both studies, the safety profile of elecoglipron was favorable. Adverse events were primarily gastrointestinal, generally mild to moderate, and included nausea, constipation, diarrhea, and vomiting. Discontinuation rates due to these events were low, and no liver safety concerns were identified. Hypoglycemia was rare in the diabetes study, with no serious cases reported. The upcoming Phase 3 development will include the EMBOLD trials for obesity and overweight populations, and the ELUMINATE studies, which will evaluate elecoglipron both alone and in combination with dapagliflozin for Type 2 diabetes. Future investigations will also explore long-term cardiovascular and kidney outcomes.

These significant advancements by AstraZeneca highlight the potential of innovative oral treatments to address widespread health challenges.
